Sleep Europe 2026 | Implementing CSA guidelines in clinical practice: a stepwise approach to treatment

Winfried Randerath, MD, PhD, Bethanien Hospital, University of Cologne, Solingen, Germany, discusses practical steps for implementing central sleep apnea (CSA) guidelines in clinical practice. Prof. Randerath explains that, as no treatment has been shown to improve survival, the decision to treat a patient should be based on symptoms and patient outcomes, with treatment starting with continuous positive airway pressure (CPAP). He highlights that CPAP often fails, in which case patients can switch to adaptive servo-ventilation (ASV). If both fail, options include oxygen, carbonic anhydrase inhibitors or interventional treatment. This interview took place at Sleep Europe 2026 in Maastricht, the Netherlands.
