Sleep Europe 2026 | Personalizing COMISA treatment: principles guiding therapy selection

Alexander Sweetman, PhD, The University of Western Australia, Perth, Australia, discusses factors guiding treatment selection in comorbid insomnia and sleep apnea (COMISA). Dr Sweetman explains that COMISA is complex and heterogeneous, and few trials have addressed whether cognitive behavioral therapy for insomnia (CBTi) or positive airway pressure (PAP) should come first. He outlines that treatment selection should be guided by patient preferences and goals, clinician discretion, and pragmatic factors such as treatment availability and cost. He also highlights that an international position statement on personalizing COMISA treatment is in preparation. This interview took place at Sleep Europe 2026 in Maastricht, the Netherlands.
