AAN 2024 | Endovascular thrombectomy and neuroprotection in large core stroke

Hazem Shoirah, MD, from Icahn School of Medicine at Mount Sinai, New York, NY, discusses ongoing research in the large core infarction space. Patient selection is one such area, with further research needed to determine the ideal imaging modality for treatment decision making and the effect of penumbral tissue on outcomes. Several trials have also been conducted to investigate neuroprotection strategies to prevent reperfusion hemorrhage in large core infarctions. One such strategy found to be beneficial for these patients is endovascular cold saline infusion for local hypothermia. Dr Shoirah also emphasizes the requirement for further research into EVT in patients with a very low (0-2) ASPECTS, as published trials focus on patients with ASPECTS 3-5 who have a higher likelihood of favorable outcomes. This interview took place at the American Academy of Neurology (AAN) Annual Meeting 2024 in Denver, CO.
