AAN 2023 | The best approach to neural antibody testing for autoimmune encephalitis

Alfonso Sebastian Lopez, MD, Mayo Clinic, Jacksonville, FL, discusses the best approach to testing for neural autoantibodies in patients with suspected encephalopathy. Utilizing reliable laboratories experienced in testing for these antibodies is extremely important. These laboratories should be able to perform tissue immunofluorescence assays and advanced diagnostics using cell-based assays and other research methodologies. It is crucial to test both the serum and cerebral spinal fluid to accurately detect the presence of classified or unclassified autoantibodies. Rather than using standard antibody panels, Dr Lopez recommends a phenotype-driven evaluation, ordering specific tests based on the patient’s diagnosis and suspected anatomical encephalopathy. This interview took place at the American Academy of Neurology Annual Meeting 2023 in Boston, MA.
