WSC 2022 | Optimal surgical timing in patients with ICH

Shahid Nimjee, MD, PhD, The Ohio State College of Medicine, Columbus, OH, discusses the optimal timing for surgical evacuation of intracerebral hemorrhage (ICH). A large number of clinical trials are now focusing on the 24-hour window after the onset of ICH or after presentation to a hospital. A hemorrhage can not only push on the brain but can cause an inflammatory cascade that results in a secondary edema, typically potentiating at around 72 hours after ICH onset. The surgical evacuation therefore aims to remove the clot before a secondary edema can occur and in doing so, provide the patient with the greatest clinical benefit. This interview took place at the World Stroke Congress 2022 in Singapore.
