ISC 2026 | Atrial cardiomyopathy as a potential embolic source in large vessel occlusion: a case report

Hiroshi Yamagami, MD, PhD, Osaka National Hospital, Osaka, Japan, shares a case report on atrial cardiomyopathy as a potential embolic source in large vessel occlusion. Prof. Yamagami reports that this patient experienced a recurrent stroke with an undetermined cause, and fibrosis and mild amyloidosis were found in the atrial tissues during post-mortem examination. This interview took place at the 2026 International Stroke Congress (ISC), held in New Orleans, LA.

This is a case report. The patient who suffered an undetermined cause of stroke. And this patient had a previous stroke with undetermined ischemic stroke and we tried to find an embolic source for two years with the intracardiac monitor but AFib had not been detected. After then the patient had recurrent stroke with a large vessel occlusion and after the second attack we found just a two-second AFib episode. And after half a year the patient died with pneumonia and that pathological check was done and the atrial tissues had shown fibrosis or amyloidosis, mild amyloidosis was found in the pathological findings so we suggest this patient has an atrial cardiopathy which caused the stroke, embolic stroke in two episodes.
