ISC 2026 | The I-CATCHER study: implementing a bundle of care for intracerebral hemorrhage

Valeria Caso, MD, PhD, FESO, Santa Maria Della Misericordia Hospital, University of Perugia, Perugia, Italy, discusses the I-CATCHER study (NCT06429332), an implementation project based on the INTERACT3 trial (NCT03209258), which demonstrated that a bundle of care can improve outcomes in intracerebral hemorrhage. Dr Caso highlights that the project aims to apply this bundle of care in high-income countries, starting with mapping and cluster randomization of centers. This interview took place at the 2026 International Stroke Congress (ISC), held in New Orleans, LA.

The I-CATCHER trial is based on the results of the INTERACT3 trials that a bundle of care can improve the outcome in intracerebral hemorrhage patients. This INTERACT3 trial was performed in low and middle income countries. So this time we see that you can reproduce in high income countries. And it’s important to apply the bundle of care to intracerebral hemorrhage patients. So what we are now doing is creating batches. So it’s not a real study. The I-CATCHER study is an implementation project because we realize that we, even in our high-income countries, do not apply what is the bundle of care for our hemorrhagic patients. I did the work in my institution. I was shocked how bad we were in treating our patients. So this is a real structured implementation project. So we start first with mapping to understand what is going on in our institution. Then there is a cluster randomization of the centers to be high intensive bundle of care applied treatment compared to not high bundle of care treatment. Now currently in Italy we are in the phase of creating different batches. So we are in the observation phase. In Sweden, they have almost around more than 50 centers. They are already quite forward in batching the different centers. So now we are slowly moving forward, but steadily moving forward.
