ESOC 2024 | IVT before EVT in posterior circulation occlusions

Robrecht Knapen, MD, Maastricht University Medical Center, Maastricht University, Maastricht, Netherlands, outlines his research into the efficacy of intravenous thrombolysis (IVT) for ischemic stroke in the posterior circulation. Guidelines for the anterior circulation are much more comprehensive and the limited research available into the posterior circulation means that although the same standard of care is used, it is less informed. Dr Knapen investigated the use of IVT prior to endovascular treatment in posterior circulation occlusions in the MR CLEAN prospective, multicenter registry. No differences in clinical outcomes and safety were found between patients treated with or without IVT before EVT, which aligns with anterior circulation studies. Dr Knapen emphasizes that future analyses focusing on the effect of IVT in the posterior circulation are necessary to determine if the positives of IVT outweigh the rnegatives in these patients. This interview took place at the 10th European Stroke Organisation Conference in Basel, Switzerland.
